Serious Games and ADHD Screening
Potential, evidence, and important limits
A serious game uses game mechanics for a purpose beyond entertainment, such as education, rehabilitation, training, or structured measurement. In child health, a playful task may reduce the feeling of being tested and can record detailed interaction data. That potential is valuable—but it does not make every game a validated health tool.
What a game-based task can observe
Depending on its design and technical quality, a digital task may record:
- accuracy: correct responses, missed targets, and responses to non-targets;
- timing: reaction time, variability, and changes across the session;
- interaction patterns: pauses, repeated actions, and task completion;
- movement signals: accelerometer or gyroscope data when collection is justified and consented to; and
- contextual information: device type and session conditions needed to judge data quality.
Why engagement matters
A child-friendly story, understandable feedback, and age-appropriate interactions can help a child complete a task. Better engagement may improve completion and data quality, but it can also introduce new influences: familiarity with games, motor skill, visual design, rewards, device performance, and excitement can all affect results.
From a promising prototype to a trustworthy tool
- Usability and safety: can children understand and complete the experience without unacceptable burden?
- Technical reliability: are events, timing, and sensor measurements captured consistently across supported devices?
- Measurement reliability: are relevant scores sufficiently stable when conditions are controlled?
- Validity: do scores relate to the intended construct and add useful information beyond existing observations?
- Screening performance: in a representative, pre-specified study, how often does the tool produce false reassurance or unnecessary concern?
Gamification describes the experience. Validation establishes what an output can responsibly mean. They are not the same thing.
What serious games cannot do alone
ADHD assessment is multi-source and clinical. A game cannot independently establish onset, behavior across settings, developmental expectations, impairment, or alternative explanations. It should never be used by itself to diagnose, exclude a condition, change treatment, or make a high-stakes educational decision.

- Was the exact released version studied independently?
- Who participated, and does that population resemble the intended users?
- What was the reference assessment, and were evaluators blinded?
- Are uncertainty, missing data, and false-positive and false-negative results reported?
- How are child data, consent, retention, deletion, and security handled?
- Does the product clearly explain that it is not a standalone diagnosis?
